The ligaments are often preserved or substituted—not removed.
Many people assume the entire knee joint, including all ligaments, is taken out during TKR. In reality, the posterior cruciate ligament (PCL) is often preserved in cruciate-retaining implants, or replaced by the implant’s design in posterior-stabilized implants. This helps maintain more natural knee movement after surgery.
